The assignment of benefits form was created by Medicare. This form allows a durable medical equipment company to charge a Medicare recipient for a piece of equipment that is considered a non-covered item for the reasons listed below:
 
-       Item is not considered medically necessary.
-       The patient has already received same or similar piece of equipment within a valid period.  For example:  Medicare only allows one Nebulizer rental every 5 years.
-       The patient doesn’t meet the requirements for the equipment or they do not have an order but still wish to purchase.
